FEES

I am considered out of network for the purposes billing. I accept debit or credit card for services. It is necessary that a form of payment be on file to book an appointment.

My session fees range from 300-375 per session.

At your request I will provide you with a monthly statement with diagnosis and appointment codes which you can submit to your insurance company for reimbursement if you have coverage for out of network services. I recommend calling your insurance company to verify benefits. 

When you talk to your insurance company, I recommend asking about reimbursement rates for the following codes:

CPT 90792: 50 minute initial evaluation

CPT 99214: 30 minute psychopharm appointment

CPT 99214 + 90833: 45-50 minute therapy appointment with psychopharm services

CPT 90834: 45-50 minute therapy appointment without psychopharm services

LOCATIONS SERVED

For telemedicine appointments, clients must by in California or Massachusetts at the time of the appointment due to state medical license restrictions. 

CANCELLATION POLICY

Sessions must be canceled 48 business hours in advance to avoid incurring a charge. Missed sessions are billed at the full fee.

REFILLS

Please allow two business days for refill processing in the event you need a refill before our next scheduled appointment.
